Additional Instructions for Ballot Option:
Following is a list of issues prepared based on the level of importance reported
to date. Please cast Option A, only if all issues are acceptable as is, and
place a "check" mark in front of each item. Cast Option B if disagreeable
with at least one item and/or sequence of importance as listed is not acceptable.
Cross out the issue(s) not acceptable and rearrange the list if necessary. Cast
Option C only if disagreeable with the entire Ballot Item #1, and provide explanation
in the space provided.
