This collection of information is voluntary and will be used to accept nominations for the collection for FHWA's Excellence in Right-of-Way Awards Program and Utility Relocation and Accommodation Awards Program Public reporting burden is estimated to average 6 hours per response, including the time for reviewing instructions searching existing data sources, gathering and maintaining the data needed, and completing and reviewing the collection of information. Please note that an agency may not conduct or sponsor, and a person is not required to respond to, a collection of information unless it displays a currently valid OMB control number. Nomination Instructions

Nominations for the Federal Highway Administration (FHWA) 2014 Excellence in Utility Awards will be accepted from October 1, 2013 through midnight December 18, 2013, and must be submitted electronically to emily.futcher@dot.gov. Nominations must include this form, a written endorsement by a State Department of Transportation, and at least one photograph (.jpg or .gif). Once submitted, photographs become the property of FHWA.

Nominations will be rated by a panel of transportation and utility professionals on how successfully the nominations exemplify the criteria established for each category. Nominations will also be evaluated by the extent to which they accelerate project delivery, reduce costs, and improve utility collaboration and cooperation.

To nominate a project, team, or individual for an award, please follow the instructions below:

  1. In Section I, fill out your contact information.
  2. In Section V, submit at least one photograph (.jpg or .gif) and include a brief description.

For nominations related to the Program Development, Construction Management, Innovation, and Incentives Relocation award categories, in addition to instructions 1 and 2 above, follow instructions 3 and 4 below:

  1. In Section II, fill out the contact information of the organization(s) you are nominating for the awards.
  2. In Section III, provide a description of the nominated project. Check the box next to each award category for which you would like to nominate the project (up to four). For each category selected, describe how the project addresses the nomination criteria for that category.

For nominations related to the Utility Program Leadership category. Note there are two separate options within the Excellence in Utility Program Leadership Award category, Individual and Team. Nominations will be accepted for either option. Follow the instructions below:

  1. To nominate an individual: In addition to instructions 1 and 2 above, fill out Section IV (A). Provide the individual's contact information and a description of their achievements as they relate to the award category.
  2. To nominate a team: In addition to 1 and 2 above, fill out Section IV (B). Provide team members' contact information and a description of their achievements as they relate to the award category. Space has been provided for up to four team members. Use the description space to nominate additional team members, if needed.

Categories

Project Development: For a specific transportation project that met the following criteria:

  • Planned and designed a complex project which involved the relocation or accommodation of utilities;
  • Integrated policies, procedures or non-traditional approaches to improve utility development and design of a project;
  • Employed an exceptional collaborative process with utility stakeholders throughout the project development process;
  • Successfully kept the utility-related activities off the "critical path" of the project; and
  • Completed the design of the project on or ahead of schedule and within budget.

Construction Management: For a specific transportation project that met the following criteria:

  • Provided management and coordination related services in support of relocating or accommodating utilities during the construction of complex projects;
  • Employed a collaborative process which involved utility stakeholders throughout the construction of the project;
  • Used innovative (e.g., construction contract provisions) or non-traditional approach to address a particular utility relocation or accommodation challenge encountered during construction; and
  • Successfully avoided or minimized contract modifications/change orders resulting from utility related challenges encountered during construction.

Innovation: For a specific transportation project that met the following criteria:

  • Incorporated a new or innovative technology, process (e.g., master agreements) or practice (e.g., partnering agreements) during the planning, design or construction of a project to facilitate the relocation or accommodation of utilities in a more timely and cost effective manner;
  • Incorporated a new or innovative technology, process or practice that significantly enhanced the administration, management or services of an agency or organization's utility program.

Incentives for Utility Relocation: For a specific transportation project that met the following criteria:

  • Used financial and/or non-financial incentives or reimbursement techniques to accomplish utility-related design or construction activities on time or ahead of schedule.

Utility Program Leadership: The team or individual who met the following criteria:

  • Demonstrated exceptional team-building and consensus-building skills to advance the utility discipline at their agency, organization or nationally;
  • Advocated research and technology development and/or deployment that benefited the utility discipline;
  • Implemented policies or initiatives that better integrated the consideration of utility relocation and accommodation issues within the project planning, design and construction processes at their agency or organization.
