Possess all relevant Materials Safety Data Sheets and be familiar with the information they contain.___

Ladders and scaffolding are in good condition and provide safe access to all necessary areas. Any needed changes should be immediately brought to contractor’s attention.___

If working at heights greater than 2m (6 ft.), proper fitting safety harnesses or belts have been provided.___
Comments:




 

During blasting operations, no dust leaks are visible outside the enclosure. If working inside the enclosure, appropriate breathing apparatus has been provided.___

Once blasting is completed, there is no lead debris is present on the ground or in any waterways.___
Comments:




 

Hazardous waste is stored away from public access and in an area where it is not subject to being struck by traffic.___
